Should you decide to continue with a new enrollment, your information must be shared with the

insurance carrier that you have selected. Your eligibility for Medicare or Medicaid would be

verified using systems approved by CMS and DHS Medicaid programs with your consent.

Legal Reasons

At the request of any enforcement agency, rather it be Medicare, Medicaid, or a judicial

request, we will abide by and follow all local, state, and federal laws required to comply with

the request. You will always be notified if your information has been requested and PHI shared

beyond the scope of conducting normal business.

How Long We Store Personal Data

User data will be stored for 12 months then archived. If the consult results in a new enrollment,
